Team Registration Agreement

WAIVER AND RELEASE OF LIABILITY READ BEFORE SIGNING

1. Team representation and player consent. I, as the coach and/or authorized team representative, confirm that I am authorized to register the team for the Charlotte Rise FC Winter Mini Futsal League and related league activities. My responsibilities under this agreement apply to every player I bring to the league, including players added after registration and guest or substitute players. Before each player participates, I am responsible for obtaining approved participation consent, liability acknowledgment, and medical authorization from that player’s parent or legal guardian, retaining them, and providing them to Charlotte Rise FC upon request.
2. Acknowledgment of risks. I recognize the possibility of serious injury associated with futsal and acknowledge the risks and hazards related to my team’s participation in the league and related activities. I accept these risks for my own participation and will communicate them to players and their parents or guardians.
3. Waiver and release of liability. To the extent permitted by law, I release, discharge, and hold harmless Charlotte Rise FC and its affiliates, sponsors, employees, coaches, representatives, and agents from claims that I am legally entitled to release arising from my participation and my team’s participation in the league and related activities. I further agree, to the extent permitted by law, to indemnify and hold harmless these parties from claims to the extent caused by my failure to fulfill my responsibilities under this agreement.
4. Medical care and emergency contacts. I am responsible for maintaining signed parent/guardian medical authorizations and current emergency contact information for every player. If medical attention is needed, I will promptly seek assistance and help Charlotte Rise FC staff contact the player’s parent or guardian and arrange care consistent with those authorizations and applicable law. I understand that Charlotte Rise FC does not assume financial responsibility for players’ medical treatment.
5. Player needs and restrictions. I am responsible for informing Charlotte Rise FC, with parent/guardian permission, of any medical needs, activity restrictions, or accommodations that may affect a player’s safe participation, and for communicating any updates before that player participates. I agree to release, indemnify, and hold harmless Charlotte Rise FC, its employees, and coaches from claims arising from incomplete, inaccurate, or undisclosed information, or from Charlotte Rise FC’s good-faith efforts to support the needs I have disclosed.
6. Photo and video release. Charlotte Rise FC representatives may capture photographs and videos during league games and other league activities. These materials may be used for marketing purposes, including but not limited to the Charlotte Rise FC website, social media platforms, electronically distributed club materials, and promotional videos. My team’s participation in Charlotte Rise FC’s Winter Mini Futsal League and related league activities constitutes acknowledgement and acceptance outlined in the release.
